In this procedure, the surgeon creates a cut in the patient's back and then injects a narrow telescope into the kidney. The surgeon will then break the stone up and take it away. This process is used in cases of huge stones or in the event that the lithotripsy does not work.

Although rarely used, oftentimes the surgeon also can make a cut in the side in order that he can reach the kidneys. This permits him to easily remove the stone. This kind of kidney stone treatment is scarcely needed.
